The consent form
This should include the following items.
The participant:
- has read and understood the information
- consents to be interviewed/ observed/ or take part in any other method (each item should be given a new line and each aspect be initialled
- agrees to recording (visual or voice)
- can withdraw from the study at any time without giving a reason
The researcher should realistically identify how data collected will either be included or withdrawn (with rationale).
The document should be signed and dated. A copy should be given to the participant and another be retained by the researcher.
