Long term conditions pathway
Prioritising the care of those with long term conditions (LTCs) has become a key focus for the Department of Health and other agencies over recent years. People with LTCs are intensive users of primary and secondary care services and there are huge benefits to be gained from effective chronic disease management and the delivery of high-quality, personalised care.
Within the BSc(Hons) Professional Practice degree there will be provision for nurses working in a variety of professional contexts in both primary and secondary care, as well as those working in non-NHS care environments, to develop the knowledge and skills necessary to care for those with LTCs. The LTC pathway will enable practitioners who wish to 'major' in this area of clinical practice to access modules relevant to their existing and/or developing roles.
Module choices will be discussed with the pathway leader to ensure that 40 level 3 credits are gained from the LTC pathway modules and that, after completing the research module (20 credits), the dissertation (30 credits) undertaken also focuses on LTCs. Practitioners may then choose any module / modules available in the undergraduate CPE provision to make up the remaining 30 credits for the Professional Practice degree.
