You use this form to confirm you are making a Clearing application to us. All you have to do is sign it electronically, send it back to us and we will notify UCAS on your behalf.

The Confirmation of Clearing Application form is an interactive PDF, which means you can type directly into it and send it back to us by email – but make sure you download and save a copy before you start.
