Applied clinical and practice-based research

The aim of the applied clinical and practice-based research for midwives, nurses, occupational therapists, paramedics, physiotherapists and podiatrists research area, is to foster excellent research relevant to allied health professionals’ practice and to underpin clinical reasoning, diagnosis, treatment approaches and self-management strategies in patient populations within the remit of nursing, midwifery and the allied health sciences. Research takes place in clinical, laboratory and other health and social care settings.

The research themes are:

  • Musculoskeletal therapy and rehabilitation
  • Exercise prescription and health
  • Posture and mobility
  • Efficacy of therapeutic interventions/approaches

The applied clinical and practice-based research area covers the research interests of all staff and doctoral students with research interests in this area. As such the area fosters research into clinical practice and effects of treatment modalities, the therapeutic relationship, patient views in relation to conditions and treatment received as well as issues relating to practitioners. The area is also interested in the field of therapeutic exercise. Research can be based in clinical or laboratory settings and can involve qualitative as well as quantitative approaches.

We provide a forum for discussion of research activities, for sharing expertise in methods and research approaches and to offer support to all those undertaking or wishing to undertake research in this area. We support all members associated with the area in their research activities, help colleagues with conference presentation preparation, with research planning and publication activities as well as grant applications. We have links with clinicians and other academics at the University of Brighton and also invite doctoral students to attend the discussion areas. The profile of the area includes midwives, nurses, physiotherapists, podiatrists, occupational therapists, sports scientists and osteopaths. We also collaborate with staff from other parts of the university, eg. Brighton and Sussex Medical School and with departments in other universities together with clinicians and managers locally.

We meet on a regular basis for informal discussions and supportive activities and on an annual basis for strategic planning. We value all personal input from researchers and those interested in becoming researchers, and those who are interested to find out more about research and share ideas and knowledge.

Some people say: ‘I have got a really bad backache’ and you think ‘Okay, yes, backache'. Until you have actually had low back pain you don’t really know what it means.

Musculoskeletal therapy and rehabilitation

Our research largely focuses on increasing understanding of the physiological and functional mechanisms underpinning low back pain, evidencing treatment and management approaches to low back pain and increasing understanding of the experience of low back pain from the patients and clinicians’ perspectives. To this end we engage with laboratory-based research, clinically-based research which includes quantitative and qualitative methods together with mixed methods approaches.

Podiatry

Podiatry as a profession seeks to assess, diagnose and manage lower limb complaints, typically in a multi-disciplinary context. As such the research the Podiatry Division engages with encompasses all the research areas within the School of Health Sciences and links with other university research units (e.g. Centre for Regenerative Medicine and Devices) as well as National and International collaborations.

We conduct research in a range of fields where foot complaints are particularly common and debilitating such as diabetes, musculoskeletal disorders and circulatory disease. Our recent focus on the ‘at risk’ foot involves both quantitative and qualitative paradigms and seeks to enhance our understanding of the incidence and prevalence of foot complaints, their underpinning pathophysiology and the impact of these both at an individual and societal level: together with how foot complaints are best managed.

Osteopathy

The Osteopathic Research sub-theme developed within the CHR during Professor Moore’s tenure (2005-2012) as Chair of the National Council for Osteopathic Research (NCOR) supported by Carol Fawkes as Research Officer for NCOR, Shirley Mathias as NCOR Research Administrator, Dr Janine Leach as Senior Research Fellow in Osteopathy, and project-funded research assistants. The group developed research capacity in osteopathy, with a number of osteopaths who worked within or with the group on research projects or who undertook research training. The outputs from the group included osteopathic presentations and publications, and research collaborations with the osteopathic training colleges, the regulatory body and the professional body. This period saw the completion of several significant funded studies, and the initiation of new studies within osteopathy.

Practice-based osteopathic research

The publication of small trials or case reports by practitioners can provide valuable preliminary data on which to base further research. These studies have included a survey on the role of osteopaths in supportive care of cancer patients by Janine Leach; collaboration with Mark Matthews on studies of osteopathy and dyslexia; and collaboration with Dr Rod MacDonald and Dr Theo Peters on a pilot trial of osteopathic management of restless legs syndrome. The protocol for the latter has been published in International Journal of Musculoskeletal Medicine.

Professor Ann Moore ended her term as Chair of NCOR in May 2012, however the osteopathic group based at the CRC has continued to develop osteopathic research. During 2013-2014 new research included a project to explore osteopathic research priorities using a Delphi methodology, and a Cochrane systematic review to provide evidence on a phytonutrient for improving quality of life in cancer survivors.

Research developments have also included PhD work by Dr Oliver Thomson, supervised by Dr Nikki Petty and Professor Ann Moore. The focus of this work has been on clinical reasoning in osteopathic private practice.

Posture and mobility

Researchers continue to progress the research in the field of posture, mobility and assistive technology. The Neater Uni-wheelchair development continues to attract national and international interest.  

The Posture and Mobility group continues to support their professional doctorate and PhD students. The outcomes from all the work undertaken by the posture and mobility group are beginning to have national impact on clinical practice.

The research emanating from this group fulfils the DoH national priority of meeting the needs of the disabled and also patient and public involvement. The group shares ideas and provides support to all the participating students. It also advises on funding availability and helps with funding proposals. Staff and students working in any of the aforementioned areas are welcome to contribute to the group.

Consultancy Work

We undertake research consultancy with a range of partners including: Chailey Heritage Clinical Services to explore posture in standing frames, Neater Solutions, and NuDrive .

Collaborative Work

Dr Mandy, Dr Pountney and Diane Sellers were awarded a Research for Patient Benefit (RfPB) grant to develop a standardised eating and drinking
classification system. This work will also underpin a PhD study. Dr Pountney and Dr Mandy are collaborators on an RFPB funded project which is an RCT on exercise in non-ambulant children using static bikes and treadmills.
Dr Nick Webborn is a collaborator on a grant submitted to Arthritis Research Council and Promobilia.
